Patchy Hair Loss Cats

What could this hair loss mean?

I know a cat who has patchy missing fur around his neck and the base of his tail. There is scabbing and you can feel it as you pet him. He doesn’t seem miserable, but he does scratch. He does have fleas, as fleas are very difficult to get rid of in South Georgia, especially sand fleas. It has not been affecting any of the other animals. He eats and drinks regularly, and his stool is fine. He gets a regular worming for all worms as well. He is on Revolution and receives flea baths weekly for fleas.
